Whether you choose to participate in the programs is completely up to you. But remember, you matter to your family, your friends, your pets, your community, and your employer.

SET A GOAL

To improve your odds of success, set personal goals using the goal setting form. Make your goals realistic and reward yourself when you reach them. Don’t forget to take time throughout your journey to track your progress or regroup along the way.

COMPLETE MONTHLY CHALLENGES

The Wellness Program features monthly challenges to help you focus on healthy habits. New for 2018 -- Complete all 12 monthly challenges to achieve Wellness Warrior status and earn a special reward.

While a different challenge will be featured each month, you can complete them in any order you like. Multiple challenges may be completed during a month (must be different goals); however, challenges cannot be repeated for additional credit.

Challenge information and monthly tracking logs are available here.  You can track your progress using your smartphone or computer.  Just attach a printed report to the monthly tracking log. Tracking logs must be submitted by the 5th of the next month to receive credit.

REIMBURSEMENT PROGRAMS

Benefit eligible employees have the opportunity to receive retroactive reimbursement for participation in the programs listed below. Refer to the applicable reimbursement form for utilization requirements and submission deadlines.

EARN A DEDUCTIBLE CREDIT

By completing the monthly challenges, benefit eligible employees can work toward a deductible credit.

Employees can earn a $50 or $100 deductible credit depending on the programs completed. Multiple challenges may be completed during the same month (must be different goals); however, challenges cannot be repeated in separate months for additional credit.

Complete at least 5 Challenges - Earn a $50 Deductible Credit on your 2019 medical deductible

Complete at least 10 Challenges - Earn a $100 Deductible Credit on your 2019 medical deductible

In addition to the Monthly Challenges, benefit eligible employees can earn one challenge credit toward the deductible credit for the following completed programs:
