Whether you choose to participate in the programs is completely up to you. But remember, you matter to your family, your friends, your pets, your community, and your employer.

EARN UP TO A $100 DEDUCTIBLE CREDIT

During 2020, benefit eligible employees can receive a medical benefit deductible credit of $25 per quarter by participating in Wellness Programs, up to $100 total for 2021.

The first step in earning the deductible credit is completing the Know Your Numbers wellness screening.  Employees who completed a screening in 2018 or 2019 will receive full credit for this program.  To see if you are eligible for a screening, contact April Hugg at 419.354.1373.

Select the link below to view the information and tracking logs for each quarter.  You can track your progress using your smartphone or computer.  Just attach a printed report to the tracking log. Tracking logs must be submitted within seven days of completing a challenge to receive credit.

REIMBURSEMENT PROGRAMS

Benefit eligible employees also have the opportunity to receive retroactive reimbursement for participation in the programs listed below. Refer to the applicable reimbursement form for utilization requirements and submission deadlines.
