2022 Wellness Challenges & Events

 

COMPLETE MONTHLY CHALLENGES TO BE ENTERED TO WIN MONTHLY PRIZES!

Select the link above to view the program information and tracking logs for the monthly challenges.  You can track your progress using your smartphone or computer.  Just attach a printed report to the tracking log. Tracking logs must be submitted by the 5th of the month (if it falls on a weekend, the next Monday) to receive credit and be entered to win monthly prizes.

 

EARN UP TO A $100 DEDUCTIBLE CREDIT!

During 2022, benefit eligible employees can receive a medical benefit deductible credit of $25 per quarter by participating in Wellness Programs, up to $100 total for their 2023 medical deductible.

The first step in earning the deductible credit is completing the Know Your Numbers wellness screening.  Employees who completed a screening in 2020 or 2021 will receive full credit for this program for 2022.  To see if you are eligible for a screening, contact the Benefits Clerk at 419.354.1373 or email wellness @co.wood.oh.us.

In addition to the Wellness Screening, employees need to complete two monthly challenges and attend one wellness event each quarter.  See the link listed above to view challenge and event information.   Learn more in the Wellness Program Supplement to the Summary Plan Description.

 

REIMBURSEMENT PROGRAMS

Benefit eligible employees also have the opportunity to receive retroactive reimbursement for participation in the programs listed below. Refer to the applicable reimbursement form for utilization requirements and submission deadlines.
