Use this form to assist you in meeting your wellness goals.  It can help you map out the programs you wish to complete during 2016.

Monthly Challenges

Win Prizes by completing a monthly challenge.  The more challenges you complete, the more chances you have to win throughout the year!

Completed challenges must be received by the date noted on the monthly tracking log.  You can complete the challenges in any order you like.  Just use the tracking log for the current month to record your activity.  Refer to the Featured Challenge for what to track on the tracking log.

Benefit eligible employees can also work toward a deductible credit.

Complete at least 5 Challenges  - Earn a $50 Deductible Credit

Complete at least 10 Challenges - Earn a $100 Deductible Credit
